Fill in patient intake form

Let the doctor know about your health and lifestyle history. Our doctor will review your test and intake form to make sure you are testing what you need.

Age, gender and ethnicity
Symptoms and health concerns e.g. Urine changes, drug/food allergies
Family and personal medical history e.g. High cholesterol, Diabetes, liver disease
Long-term medication use
Alcohol use and smoking

Test Anywhere

At the clinic nearby your home or workplace

After pre-registration, our Care Manager will send you a QR code to show at the clinic upon arrival. You will also get a test preparation checklist if you are required to fast before doing the test.

During the testing process, we will measure your body composition, blood pressure, collect your blood, urine and/or stool samples.

Or book a home visit

Our professional phlebotomist will come to you directly to collect your measurements and samples. Our experts ensure a smooth and hassle-free sample collection process. If you are doing a stool test, we will send you a sample collection kit, so that you can collect your samples at your own convenience.
